| Eligibility of Service: |
A previously diagnosed moderate to severe mental illness, a suspected moderate to severe mental illness or have difficulties functioning in at least 2 areas. i.e. (school, home, social). |
| Referral Process: |
All referrals must come through your G.P, School Counsellor/Teacher, Public Health Nurse or other child and youth related professional. |
